What Should Actually Be in My Healthcare Directive?

By Estate Prep Partners  ·  5 min read

Naming a healthcare proxy answers the question of who speaks for you. It doesn't answer the harder question: what would you actually want them to say? Without that second part, even the person who knows and loves you best is left guessing at the worst possible moment.

A healthcare directive exists to close that gap — turning your general sense of "they'll know what I'd want" into something specific enough to actually guide a real decision.

"The person you choose only knows what you tell them. A directive is how you actually tell them."

What a Meaningful Directive Actually Covers

Life-sustaining treatment

Your general position on ventilators, feeding tubes, and other measures if there's little or no chance of meaningful recovery.

Comfort vs. extending life

Whether you'd prioritize comfort-focused care over aggressive treatment in a terminal or irreversible situation.

Organ donation

A specific, explicit preference — often assumed but rarely actually documented clearly for a proxy to reference.

Where you'd want to be

Home versus hospital, if a choice is realistically possible, given your condition.

None of this needs to cover every conceivable medical scenario. What matters is giving your proxy enough of your actual values and priorities that they can make a judgment call you'd recognize as yours, even in a situation you never specifically anticipated.

A real pattern

It's common for someone to name a proxy and assume that's the entire task done — trusting the relationship alone to guide the right decision. In an actual crisis, even a proxy who loves you deeply can freeze or disagree with other family members without a documented sense of your specific wishes to point to. The relationship matters. So does giving them something concrete to stand on.

A Reflection Before You Assume It's Covered

Check off what's true for you right now. There's no scoring here — just a clearer read on where your thinking actually stands.

What Comes Next

Whether you've already thought through your specific wishes or you're realizing you've only ever named a proxy without saying much else, instinct alone isn't quite enough to be sure they'd know what to do. Choosing the right person was only ever half the job.

That's exactly what the Healthcare Proxy Workbook inside the Estate Prep Partners system helps with: working through your actual medical values and wishes in detail, not just naming who speaks for you.

Give your proxy more than just the role
